Sir,

With due respect and humble submission, I beg to state that when, I was going back to my lodge after the office hour yesterday I met with an accident. I was proceeding to my lodge by a richshow (main cause). When I was just in front of the Nawab Altal Ali Super Market, a Scooter, which was moving to the opposite direction, suddenly dashed against my rickshaw. The rickshaw was completely smashed and I got injuries all over my body. I was immediately removed to the Anowar Hospital, word 03, bad-11, where I am lying in a miserable condition. The Physician in charge of the ward says that I shall require a fortnight’s time to recover.

For the about circumstances, I am not in a position to attend my duties and request you to kindly grant me a fortnight’s Medical leave with effect from today. I enclose herewith a Medical certificate as required under the rules.
